Blended Service Coordination

Blended Service Coordination, Allegheny County PAThe Blended Service Coordination Program is licensed through the Pennsylvania Department of Public Welfare. In order to qualify, children and adolescents enrolled in the program must meet specific DPW eligibility requirements, including:

  • Involvement in two or more child servicing systems
  • Documented evidence of behavioral health diagnosis
  • Commitment to utilizing mental health services

Service coordinators utilize client and family strengths to goals related to obtaining/maintaining mental health services, improving outcomes in educational programs increasing social opportunities, decreasing legal involvement, collaborating with physical health service, etc. The ultimate design of the program is to ensure client /families have desire natural/community/ professional supports to achieve and maintain stability and success in their community.

All clients are provided with comprehensive assessment, service planning, linkage to services, monitor service delivery, problem resolution, informal support network building, and connection to community resources. All services are provided in adherence in CASSP (Child and Adolescent Service System Program) principles.
