Service coordination for childrena and teens, Allegheny County PAThe Life Project

The LIFE Project identifies high-risk children throughout Allegheny County who require intensive behavioral health treatment in the least restrictive community environment. Referrals can be initiated by parents, service providers, educational providers or other child serving agencies.

The LIFE Project provides short tern individualized service coordination for Children/adolescents, and their families, from age 3-21. Utilizing a multi-system approach, the LIFE Service Specialist plans, implements and coordinates structured services focused on specialized service coordination, intensive behavioral health treatment and advocacy. The LIFE Project monitors all services to ensure that child and family needs are met; and will also be available to assist clients and their families in accessing resources within the client's home, school and community.

Eligibility

  • Between ages of 3 to 21
  • Children/Adolescents who have serious mental health needs
  • At high risk for placement outside of the home
